We respect your privacy. Dental practice Surinameplein strives to secure your privacy as best as possible and deals confidentially with your information. When processing personal data, we consider the applicable privacy laws and regulations. In this privacy statement we inform you about the way in which we process your data.
Categories of personal data
By using the website and the services available on it, you leave certain information with us. This also happens at our dental practice by execution of the treatment agreement. They may consist of personal data. We store and use personal data only if they are provided by you directly, or when it is clear that they are only provided to us. Depending on the service you use, we may collect the following information:

- Gender
- Email address
- Telephone number
- Information regarding your health
- The name of your health insurancy company
- The names of your other caregivers
- Time of your appointment
- Payment details
Basis for data processing
We may only lawfully process your personal data if we do so on a legal basis. We process your personal data because this is necessary for the execution of the agreement between you and us, as laid down in Article 6 paragraph 1 sub b of the General Data Protection Regulation (Algemene Verordening Gegevensbescherming; AVG). In addition, we may process your personal data for another legitimate interest, such as informing our patients about current events or changes in our services. This basis is laid down in Article 6(1)(f) of the AVG.
Data processing purposes
The personal data collected by us is used for the following purposes:
- Keeping your medical record up to date
- Sending an invoice
- Scheduling an appointment
- Carrying out a treatment
- Performing other services requested by you
Provision of your personal data to third parties
In principle, we only provide your personal data to third parties if you have given permission for this yourself. The provision of data without your consent takes place only if this is necessary to be able to perform the agreement between you and us or if a legal obligation prescribes this.
Transfer of your files
If you choose a new dentist, you can give us permission to send your patient file to them via secure mail.
Retention periods
We will not keep your data for longer than necessary for the purposes described in this privacy statement, unless this is necessary on the basis of a legal obligation. We use the statutory retention period of 20 years from the Medical Treatment Agreement Act (Wet op de geneeskundige behandelovereenkomst; WGBO) to store your medical data. In addition to the WGBO, we also take into account retention periods arising from other legislation if applicable.
Security Measures
We have taken appropriate security measures to protect your data as well as possible. These consist of the use of secure email, the use of passwords, a strict door policy, and a drafted privacy policy that is implemented by us.
